Steps to Obtaining Your California State License

Steps to obtain your Phlebotomy License:

All Steps must be taken in order as they appear.

Students will meet the State requirement of 80hrs.

*1.  Attend a Basic Phlebotomy Training Program 20 hrs. The fee for this step will be $250.    

       

*2.  Attend an Advance Phlebotomy Training Program 20 hrs. This class will provide information applicable for the National Test.   The fee for this step will be $250.

 

*3.   Attend the Extern ship Program 40 hrs. to document 50 venipunctures and 10 skin punctures on real patients.  The fee for this step will be $500.

 

The Total for the first (3) steps will be $1000.00

Please note:  Wagner Training Institute only provides steps 1-3.

 

 

Continue on to the next (2) steps:

 

This step will be provided by an approved National Company, to issue your National Test: 

 

*4.  All students need to obtain a National Certification.  (Approved by the Department of Health Services after April 2003)

Please Note:   Submit only when you have completed the previous steps

 

This will be your final step for your License:

 

*5. Submit all documentation and (2) passport size photos to the Department of Health Services-Laboratory Field Services for your Phlebotomy License.  The fee for this step will be $54.00 to (CA DHS-LFS).  You will also need High School Diploma or GED.
